Cardiovascular Institute of Michigan accepts most major insurance carriers, such as, Medicare, BCBS, HAP, Blue Care Network, Aetna, Cigna, and Cofinity. Due to the numerous products available and complexity of individual plans, it is impossible to list them all. To obtain the most current status of our physician with your insurance plan, please call our billing office at (586) 776-2792. You may also contact your insurance directly. Some plans require that services be performed within a specific network or require prior authorization. Be sure to follow your insurance plan requirements in order for your service(s) to be covered. It is the patient responsibility to obtain referral or authorization prior to your visit to our office.
Please bring your insurance card(s) with you to each visit and notify us immediately of any changes in insurance coverage. We require co-payments to be paid at the time of the visit. We accept cash, check, and most major credit and debit cards (Visa, MasterCard, Discover, and American Express). HSA and HRA account cards are also accepted. Payment by credit or debit cards can also be made by phone.
If you are not covered by insurance, we do offer self-payment options. Please call our office prior to your appointment for specific details. Payment will be required at the time of service. If applicable, we offer payment plans suited to meet your needs. Contact our patient account specialist to set up a payment plan.
