At Cardiovascular Institute of Michigan we offer a number of services right
at our office including:
| Effective July 22, 2005 Cardiovascular Institute of Michigan, PC was
granted accreditation by the ICANL (The Intersocietal Commission For The
Accreditation of Nuclear Medicine Laboratories). The laboratory is one
of the first nuclear cardiology laboratories in the United States, Canada, and
Puerto Rico to be so recognized for its commitment to high quality patient
care and its provision of quality diagnostic testing.
Participation in the accreditation process is voluntary.
Accreditation status signifies that the facility has been reviewed by an
independent agency that recognizes the laboratory's commitment to quality
testing for the diagnosis of heart disease. |

HOURS
The office hours are Monday through Friday, 8am to 5pm.
Please call to schedule an appointment,
refill a prescription(s), and to ask any questions during regular office
hours. If you have an urgent need to speak with one of the doctors after
office hours, please call our answering service at 586 693-3861.
In case of an emergency after normal business hours,
please go to the nearest hospital emergency room and tell the doctor
the name of your cardiologist.
APPOINTMENTS
When you call our office for an appointment the
receptionist will ask you for some information including your insurance card.
Please have it available. Our
receptionist will also tell you if
a referral form from your primary care physician will be needed for the
appointment. Your appointment will usually be scheduled within two weeks.
If you are experiencing chest pain, chest pressure or shortness of
breath, please tell the receptionist and she will try and accommodate you as
soon as possible.
We will make every effort to honor your appointment
time. However, unforeseen
emergencies may occasionally cause a delay.
If this occurs, your patience and understanding is appreciated.
YOUR FIRST VISIT
When you visit our office for the first time, please
bring your medical records, insurance cards, any medications you are currently
taking and a referral from your primary care physician (if necessary).
If your primary care physician has given you a slip requesting that a
specific test(s) be performed, please bring this as well.
You will be asked to complete a new patient
registration form and medical history sheet.
Please complete is as accurately as possible.
This information will assist us in helping you.
TELEPHONE CALLS
When calling our office with a medical problem or
concern, please understand that it is impossible to properly diagnose and
treat your condition without a thorough examination.
When calling our office, please tell the staff your
name (or the name of the patient) and the nature of your call. Our office
policy is to take a message and in turn speak with your doctor.
We will return routine phone calls at the end of
the patient care day. If it is a
more urgent message, our receptionist will bring your questions to the
attention of your doctor or nurse as soon as possible. You may also try
our on-line refill process.
PRESCRIPTION REFILLS
Please call the office to request prescription refills
that will expire before your next visit with the doctor.
Please be prepared to give medication name and pharmacy telephone
number to the receptionist. We
will call your prescription in before 4PM daily.
Any prescriptions called in after that time will be called the next
business day. Do not wait until the last day of your medication to request
prescription refills. The
answering service will not process requests for prescription refills after
normal business hours.
EMERGENCIES
If you have an urgent problem, please call our office
and inform our staff of the problem. We
will make every effort to schedule an immediate appointment with an available
doctor.
If you feel that your condition is an emergency
(experiencing chest pain, pressure, shortness of breath, pain or numbness in
your arms, etc.) please go the nearest hospital emergency room.
If your insurance requires a call to your primary care physician first,
please do not forget to call them.
